Yes its possible, Im doing that with 3 different servers for each realms I host.
will to try explane..
1. open up your database with navicat (or mysql query browser), locate Realmd (or what you called the it) and open the table Realmlist.
2. add another line as the one you already have there, but the first column which is "ID" need to be another value than yours, 2 for example if yours 1. then continue to fill all columns, remember that "address" should be his IP at the new line you add, and same with port. then apply the things.
3. contact your friend to change some configs in the mangos/trinity config file, not realmlist configs. he should find a line called "RealmID = 1", doesnt have to be the value 1 he has but, it should be the same value as the ID you choosed at the new line in your Realmlist database. and save the configs.
4. restart yours and his realmlist, and see if anything been added in the console. try connect to your realmlist with wow and see if the second realm now is on.
5. if its not online, you might have to unlock the mysql to allow connections from other locations than localhost. Im not good at that since I never been having problems with it, but there should be tutorials for it.
